PURCHASE AGREEMENT ADDENDUM

The following Addendum to the Purchase Agreement is between James P. Adcox hereafter referred to as the Seller, and                                                                                , hereafter referred to as the Purchaser.


This puppy is being purchased with the following pre-existing disclosed condition.

     1. Bilateral Inguinal Hernias.


It is agreed between the Seller and the Purchaser.

     1. If the Bilateral Inguinal Hernias do not self-correct by the time the puppy is spayed/neutered, and the Purchaser's veterinarian feels they need to be corrected, and the Purchaser has them corrected at the same time the puppy is spayed/neutered, and the correction results in additional charges for the Purchaser above and beyond the normal and customary charges for a spay/neuter, then the additional charges will be reimbursed to the Purchaser by the Seller.

     2. The Purchaser agrees to provide the Seller with the veterinarian's written itemized breakdown of the additional charges incurred.


By my signature, I the undersigned, affirm I have read, understand and agree to abide by the terms and conditions of this contract.

This puppy is being purchased with the following pre-existing disclosed condition.

     1. Undescended Testis.


It is agreed between the Seller and the Purchaser.

     1. If the Undescended Testis do not descend by the time the puppy is neutered, and this results in the Purchaser incurring additional charges from the Purchaser's Veterinarian, above and beyond the normal and customary charges for a neuter, then the additional charges will be reimbursed to the Purchaser by the Seller.

     2. The Purchaser agrees to provide the Seller with the veterinarian's written itemized breakdown of the additional charges incurred.

PER FLORIDA STATUTE 828.29
--------------------------------------------------------------------------------------------------------------------------------------------------------
It is the consumer's right, pursuant to section 828.29, Florida Statues, to receive a certificate of veterinary inspection with each dog or cat purchased from a pet dealer. Such certificate shall list all vaccines and deworming medications administered to the animal and shall state that the animal has been examined by a Florida-licensed veterinarian who certifies that, to the best of the veterinarian's knowledge, the animal was found to have been healthy at the time of the veterinary examination. In the event that the consumer purchases the animal and finds it to have been unfit for purchase as provided in section 828.29(5), Florida Statues, the consumer must notify the pet dealer within 2 business days of the veterinarian's determination that the animal was unfit. The consumer has the right to retain, return or exchange the animal and receive reimbursement for certain related veterinary services rendered to the animal, subject to the right of the dealer to have the animal examined by another veterinarian.

The following Purchase Agreement / Contract is between James P. Adcox hereafter referred to as the Seller, and                                                                                 , hereafter referred to as the Purchaser.

In consideration of the purchase price of $                         the Seller transfers ownership of the above puppy to the Purchaser as of the date specified below with the following conditions:

1.  The Purchaser agrees to have the puppy examined by a licensed veterinarian of their choice within five (5) days of the purchase date. If any undisclosed preexisting medical condition is found by the Purchaser’s veterinarian, the Purchaser agrees to contact the Seller immediately. The Seller then has option, prior to testing and/or treatment, of presenting the puppy to a veterinarian of the Sellers choice for a second opinion. In the event it is determined by both the Purchaser’s veterinarian and the Seller’s veterinarian the puppy had an undisclosed preexisting medical condition at the time of sale, the Seller agrees to pay for testing and/or treatment up to the amount of the purchase price of the puppy.  Initial:       

2.  If the Purchaser wishes to relinquish the puppy back to the Seller within five (5) days of the Purchase Date for failure to pass the veterinarian examination, the full purchase price and the examination fee will be refunded to the Purchaser, so long as the puppy is accompanied by it’s original Purchase Agreement, Florida Health Certificate and a Full Written Statement from the Purchaser’s veterinarian detailing the medical condition.  Initial:        

3.  If the Purchaser wishes to relinquish the puppy back to the Seller within five (5) days of the Purchase Date for any reason other than failure to pass the Purchaser’s veterinarian examination, the purchase price minus $500.00 will be refunded to the Purchaser, so long as the puppy is accompanied by it’s original Purchase Agreement and Florida Health Certificate.  Initial:         

4.  This puppy has a two (2) year guarantee against serious hereditary and/or genetic health issues which would interfere with it's ability to be a good pet or cause the new owner great expense.  If the puppy develops a serious hereditary and/or genetic health issue within two (2) years of the Purchase Date, the full purchase price will be refunded to the Purchaser. The Seller has the option of presenting the puppy to a veterinarian of the Sellers choice (at the Sellers expense) for a second opinion.  Initial:             

5.  The Purchaser agrees the puppy is being purchased as a pet and will not be bred. The Purchaser agrees to have the puppy spayed/neutered before it reaches nine (9) months of age. The Purchaser agrees to provide the Seller with proof of the spay/neuter.  Initial:
